What's inside, and why each part is there
Requirements vary by state — check your state's licensing requirements before you send this to a family.
01 · Child information
Every state licensing file starts with the child’s identifying details, and inspectors look for them first. Collecting the date of birth and start date up front is also what lets you work out ratios and room assignments without chasing a parent later.
02 · Parent, guardian and emergency contacts
You need at least one contact who can be reached during operating hours and at least one alternate for the times the parents cannot be. Licensing rules generally require the emergency contact to be someone other than the parents, which is the part most paper forms leave ambiguous.
03 · Authorized pickup
Staff need a written, current list of who may collect the child, and a name given verbally at the door is not one. Keeping this as a field on the enrollment record rather than a separate slip is what makes it easy to keep current when circumstances change.
04 · Immunization record
Immunization status has to be on file before or shortly after the first day in most states, with exemptions documented in whatever form the state accepts. This is the single most common reason a file is marked incomplete at inspection, usually because the parent meant to bring the card in and never did. 05 · Health and allergy details
Allergies, medications and chronic conditions need to be readable by whoever is in the room today, not filed in an office. Asking for the reaction and the response separately is what turns "peanut allergy" into something a substitute can actually act on.
06 · Consents and acknowledgments
Photo permission, activity and transport permission, and the handbook acknowledgment are three separate decisions, and bundling them into one signature is what makes a photo consent unenforceable when a parent objects later.
