Which Jotform plans can be used for patient data?
Only Gold and Enterprise. Those are the two plans where Jotform makes its HIPAA features and BAA available. As listed in early October 2026, Gold includes 100 forms, 10,000 submissions a month and 100 GB of storage, and Enterprise is priced by quote. The free plan excludes HIPAA features, so you can't trial a HIPAA workflow on it.
How do you turn on HIPAA in Jotform?
Upgrade to Gold or Enterprise, enable HIPAA compliance in your account, then sign the BAA from your account's Data page. Jotform's support team points customers to that page for the BAA once HIPAA is enabled.
What doesn't Jotform's BAA cover?
Jotform's BAA covers Jotform's own service, not the tools you connect to it. If responses flow into an email inbox, a spreadsheet, a CRM or another integration, each of those needs its own BAA or has to be switched off for patient forms. Check every connection before patients start submitting.
Does Jotform work for a practice with several staff members?
Ask before you buy. Jotform's own pricing FAQ describes Enterprise as its only multi-user plan, and HIPAA's Security Rule requires a unique user ID for everyone who accesses patient data. If several people in your office need to see submissions, ask Jotform how that works on Gold. For the full list of what HIPAA requires of an online form, see HIPAA-compliant online forms.
What are the alternatives for therapy practices?
For a dedicated healthcare intake tool, IntakeQ and HIPAAtizer include a BAA on their entry paid plans, and FormDr lists one on its Complete plan. If your office also renews physician orders and insurance authorizations, a GetDocsSigned healthcare account, set up by hand with a signed BAA, combines HIPAA intake forms with a renewal tracker and fax, from $199 a month per practice, with unlimited staff users.
